Healthy Connections has developed comprehensive protocols with various clinical pathways developed for each disease state. This tiered approach has been developed to meet the needs of the patient, payer and physician. Our protocols enable us to provide the patient with the appropriate level of care and ensure that we are providing care in the most cost-effective manner. Each member of the patient's care team receives a weekly report summarizing all necessary clinical information required to manage the patient's care.

Healthy Connections partners with local home care companies to deliver a comprehensive women's service. Each network provider and their employees must meet our stringent standards. All network providers are either accredited by Medicare, Medicaid or another recognized credentialing body. Using this business model enables us to often reduce the cost of providing home care to your members.

While not direct employees of Healthy Connections, these providers are part of our clinical team and we value our relationship. Many of these providers will already be in your network. All are appropriately trained, licensed and bonded.
