Healthy Connections tracks the pregnancy outcomes for patients that have received our services. This brief questionnaire not only allows us to maintain the quality of our services, but lets us know how well our therapies are working to prevent maternal and fetal problems. There are several questions on this survey about how your baby did after delivery. If you are unsure of what information the question is asking, please feel free to contact our Call Center (1-889-304-1800) during normal business hours.
