This information not only helps us to begin the patient’s care as expeditiously as possible, but also helps us to communicate more effectively with the insurance company’s case managers regarding the care to be provided for the patients.
If you have NOT heard from our office regarding the patient’s care within ONE (1) hour of sending the referral (during normal business hours), please call our Referral Hotline: 1-888-304-1800.
You may submit a referral ONLINE by completing the online request listed above. Again, if you have NOT heard from our office regarding the patient’s care within ONE (1) hour of sending the referral (during normal business hours), please call our Referral Hotline: 1-888-304-1800.
You may also submit a referral directly by calling our REFERRAL HOTLINE @ 1-888-304-1800. Our representatives/nurses will ask for the patient’s demographical information, as well as their history, and the types of services you are ordering. They will then fax you orders to be completed and faxed back to us.

You may also choose to contact your CLINICAL LIAISON. They will be happy to assist your staff in completing the Referral Process.
If Healthy Connections is OUT-OF-NETWORK with the patient’s insurance company, we will be happy to send the referral on to another homecare company at the physician’s request. For a list of our contracted insurance companies, please click here.
Once you have submitted a REFERRAL to Healthy Connections:
Please let your patient know WHICH services you have ordered for the patient, and let them know we will be contacting them regarding these services. Click here to download a printable list of services.
If Healthy Connections needs you to contact the patient’s INSURANCE COMPANY to obtain authorization, please do so the DAY the REFERRAL is submitted. Several companies do now request this.
Healthy Connections' nurses will contact the physician office to obtain PHYSICIAN ORDERS for the patient.
Healthy Connections will verify the patient’s benefits and complete the authorization process.
Our HOMECARE NURSES and CALL CENTER NURSES will then contact the patient to arrange to begin their therapy either in the home or in the hospital (if so ordered).
Physician offices will receive a START of CARE FAX from our offices within 24 Hours of care initiation.
Physicians will receive a PLAN OF TREATMENT Form by US Mail. This details the patient’s treatment plan and is required by law in the State of Texas. Please SIGN and return this to our offices (SASE) as quickly as possible.
Offices will receive WEEKLY Faxes detailing the patient’s clinical condition. Physician offices may feel free to contact our CALL CENTER at anytime, however, to check on their patient’s condition.
Physician offices may be asked to speak with the insurance company’s case managers from time-to-time to discuss a patient’s condition and receive extended authorization for services.
Physicians will be asked to SIGN & FAX discharge orders when the patient’s therapy is completed.
