Supporting High-Risk Pregnancies... at Home

Being diagnosed with a high-risk pregnancy is stressful. Fortunately, you are not alone. Every year more than 800,000 women receive this news, and we help these pregnancies result in healthy, happy babies. The medical professionals at Healthy Connections Home Care Services have been caring for high-risk pregnant mothers for more than 11 years. Healthy Connections is owned and operated by highly-trained obstetric nurses whose focus is to collaborate with your OB/GYN to provide you with exceptional medical care in the comfort of your own home. We encourage you to learn more about the benefits of home care so you can turn those initial feelings of fear and uncertainty into stories with very happy endings.

What is Healthy Connections?

Healthy Connections provides quality high-risk obstetrical care delivered by specialized nurses for management of pre-term labor, hyperemesis gravidarum (excessive nausea and vomiting in pregnancy), pregnancy-induced hypertension, gestational diabetes management and compliance coaching and education. In addition to home visits, our 24/7 Call Center means that our highly trained OB/GYN nurses are only a phone call away when a patient or family member has a question or concern. Our Call Center and web-based charting system also enables us to collaborate closely with physicians in carrying out their care orders and in monitoring and managing each home care patient. Not only does care in familiar surroundings improve patient well-being, it also provides a more cost effective solution for families and healthcare providers.

How We Help

At Healthy Connections Home Care Services, we believe that home care is more advantageous than hospitalization for well-selected pregnant patients. Healthy Connections takes direction from the patient's obstetrician in providing a personal and customized level of health services delivered in the patient's home and supported through our Call Center staffed by skilled OB/GYN nurses. The patient plays an integral role in this relationship by being receptive to our educational efforts and remaining in regular contact with our Call Center staff.

Submit Your Referrals

You and your office may submit a referral to us at any time!

If you choose to submit a referral by fax (888-321-3171), please click on the “Quick-Referral Form” [PDF] and complete as much of this as possible. This information not only helps us to begin the patient’s care as expeditiously as possible, but also helps us to communicate more effectively with the insurance company’s case managers regarding the care to be provided for the patients.

If you have not heard from our office regarding the patient’s care within one (1) hour of sending the referral (during normal business hours), please call our Referral Hotline at 1-888-304-1800.

You may also submit a referral directly by calling our Referral Hotline at 1-888-304-1800. Our representatives/nurses will ask for the patient’s demographical information, as well as their history, and the types of services you are ordering. They will then fax you orders to be completed and faxed back to us.

Our fax number is: 888-321-3171.

You may also choose to contact your clinical liaison. They will be happy to assist your staff in completing the referral process.

If Healthy Connections is out-of-network with the patient’s insurance company, we will be happy to send the referral to another home care company at the physician’s request. For a list of our contracted insurance companies, please click here.

Referral Process:

Once you have submitted a referral to Healthy Connections:

  • Please let your patient know which services you have ordered for the patient and advise them that we will be contacting them regarding these services. Click here to download a printable list of services.
  • If Healthy Connections needs you to contact the patient’s insurance company to obtain authorization, please do so on the day that the referral is submitted. Several companies now request this.
  • Healthy Connections' nurses will contact the physician office to obtain physician orders for the patient.
  • Healthy Connections will verify the patient’s benefits and complete the authorization process.
  • Our home care nurses and Call Center nurses will then contact the patient to arrange to begin their therapy, either in the home or in the hospital (if so ordered).
  • Physician offices will receive a start of care faxed from our office within 24 hours of care initiation.
  • Physicians will receive a plan of treatment form by US Mail. This details the patient’s treatment plan and is required by law in the State of Texas. Please sign and return this to our offices (SASE) as quickly as possible.
  • Offices will receive weekly faxes detailing the patient’s clinical condition. Physician offices may feel free to contact our Call Center at anytime, however, to check on their patient’s condition.
  • Physician offices may be asked to speak with the insurance company’s case managers from time-to-time to discuss a patient’s condition and receive extended authorization for services.
  • Physicians will be asked to sign & fax discharge orders when the patient’s therapy is completed.
